GETTING READY FOR THE VISIT

Getting ready for a visit is quick and easy as 1, 2, and 3!

1. An authorized individual must be on the premises and the pet must be ready to be examined (of course!)
2. A well-lit area of your home must be cleared (i.e. a kitchen or dining room table for small pets or a sparsely furnished area with a tile floor for a larger pet)
3. All new clients must fill out and send a NEW CLIENT REGISTRATION FORM and sign AUTHORIZATION and AGENCY DECLARATION prior to visit. Also send or have ready a copy of pet(s) medical records at the time of initial visit. This allows us to become familiar with any previous medical issues.

Because your time is valuable, we will do everything possible to be prompt for scheduled appointments. In the event of unforeseen circumstances, you will be contacted regarding unexpected delays.


You may request an estimate prior to visit. If more services are provided, additional fees will apply.
